New Student Enrollment Grades 1-12
To register for grades 1–12 at Holy Family Catholic Schools, please complete all ten steps below. Steps 1–4 must be completed to hold your spot on next year’s class lists. Steps 5–10 will need to be completed before your child begins school. Please submit all hard-copy or PDF forms to your school office.
If your child is enrolling mid-year or after the official start of school, please complete steps 1-4. You will receive additional information regarding steps 5-10 once the office has received and processed your registration paperwork.
Steps 1–4: Enroll for Next School Year
Step 1: Fill out a New Family Enrollment Form. (pdf)
Step 2: Fill out the Ethnicity Form. (pdf)
Step 3: Fill out the Request for Student Records and Information. (pdf)
Step 4: Submit a $50 nonrefundable tuition deposit for each child. Checks can be made payable to Holy Family Catholic Schools.
Steps 5–10: Submit Supplementary Registration Documents Before School Begins
You will be notified in July before the new school year begins when materials are available to complete steps 5–8. If your child is enrolling mid-year, you will receive an email to complete the following e-registration steps once your registration paperwork is processed.
Step 5: If applicable, complete the Medication Request Form. (pdf)
Step 6: If applicable, complete the Transportation Reimbursement Form. (pdf)
Step 7: If applicable, complete the Free and Reduced Meals Application Form. (pdf)
Step 9: High School Only Complete the Dental Screening Form (pdf) (ninth grade only)
