To register for kindergarten at any Holy Family elementary school, please complete all eight steps below. Steps 1–3 must be completed to hold your spot on next year’s kindergarten class lists. Steps 4–9 will need to be completed before your child begins school. Please submit all hard-copy or PDF forms to your school office.
Steps 1–3: Enroll for Next School Year
Step 1: Fill out a New Family Enrollment Form. (pdf)
Step 2: Fill out the Ethnicity Form. (pdf)
Step 3: Submit a $50 nonrefundable tuition deposit for each child. Checks can be made payable to Holy Family Catholic Schools.
Steps 4–8: Submit Supplementary Registration Documents Before School Begins
You will be notified in July before the new school year begins when materials are available to complete steps 4–9.
Step 4: Submit your child’s birth certificate, baptismal certificate and medical forms packet. If you did not receive a medical packet within your kindergarten toolbox upon submitting your enrollment forms, please contact Rebecca Mueller, director of enrollment, at email@example.com to receive one.
Step 6: Complete the Single-Check Payment Form per enrolled site.
Step 7: If applicable, complete the Medication Request Form.
Step 8: If applicable, complete the Transportation Reimbursement Form.
Step 9: If applicable, complete the Free and Reduced Meals Application Form.
