Here you will find the instructions and resources you'll need to register and prepare for the new school year. Please complete registration steps 1–5 by Friday, Aug. 4.

Registration Steps for K-12 Families (steps 1–5)

PowerSchool registration opened in April, so for those who have already completed the required annual registration forms, there is no further action needed here!
For those who have not, please log in to PowerSchool to complete the required forms for each of your K–12 children by Friday, Aug. 4.
Instructions:
Option 1: PowerSchool Public Portal [desktop]:
- Log into your parent PowerSchool account using your username and password.
- Click Forms in the left navigation column to complete the 2023-2024 registration forms.
Option 2: PowerSchool Mobile App:
- Download the PowerSchool App on a mobile device and enter the following code to connect to Holy Family on PowerSchool: NZGG
- Touch More in the lower-right corner.
- Touch Forms to complete the 2023-2024 registration forms.
There are three sections of forms for review, and all forms must be completed in each section:
- New Students Only - The Home Language Survey (HLS) form must only be completed once for new students.
- Review & Update - These forms may have already been completed in past years but should be reviewed to ensure accurate information is on file with the school. Please note, they will be marked "Approved" for existing students and will update to "Pending Review" if changes are made. Please be sure to submit any changes to your address or contact information.
- Required - These forms must be completed each school year.
Important:
- Contacts - It is critical to ensure contact information for parental and emergency contacts is accurate. Mailing lists and notifications are pulled from the information in the Student Contacts portion of this form.

- Tuition statements and payment schedules will be sent via email from FACTS by the end of summer.
- For the 2023-2024 school year, returning families will be re-enrolled in their 2022-2023 tuition payment plan. If you are a new K-12 family, you will receive instructions on accessing your tuition account when statements are sent.
For those approved for an Education Savings Account (ESA):
- You must log in to your Odyssey account on or after July 15 to add your school.
- If you do not add your school via Odyssey (on or after July 15), your ESA funds will not be applied to your tuition statement.

Fee payments will now be made via ParentSquare! Select your school below to pay all non-tuition school-related fees such as Parent Association dues, yearbooks, and more. If you prefer to make a check payment, please stop by your school or Central Office to make a payment.

Physical exam records are required for secondary students who participate in athletics. Elementary students (K–5) are required to submit a Health Records Form. All students should provide up-to-date immunization records. Forms can be returned to your school at orientation or by email to bbroderick@holyfamilydbq.org.

- Athletics concussion form (grades 7-12 – REQUIRED FOR ALL ATHLETES)
- Dental screening form (Kindergarten and 9th-grade only)

PLEASE NOTE: All incoming seventh-graders must have the TDap and Meningococcal vaccines and 12th-graders must have the Meningococcal vaccine before school starts.

- Visit the Meal Magic Family Portal, to view lunch balances, make payments, transfer funds between children, view meal purchase details and set account restrictions. Payments can also be made by sending a check to school with your child.
- If your family will need help paying for school meals this year, please fill out a Free & Reduced Meals application.
- If your student requires a special diet, please fill out a Diet Modification form.
- Did you know you can access the daily lunch menus on ParentSquare? Join the Elementary, Mazzuchelli and/or Wahlert Lunch group(s) to access the daily lunch calendar or even subscribe to the calendar to sync it with your personal device!

Summer uniform and spirit apparel orders through the Holy Family e-Store will be placed on July 24 and August 10.
Visit the Uniforms page for dress code information and to shop for uniform-approved items featuring Holy Family official logos. Items can be purchased through the Holy Family eStore or Lands’ End School. Crested (Holy Family branded) polos are only required at the middle and high school levels but are available for all schools.
Uniform samples (and Lands’ End Uniform Fit Kits) are available at the Holy Family Central Office at 2005 Kane St, Dubuque, Monday through Friday, 8 a.m. to 4 p.m.
Uniform Recycle Event Happening Now
Stop by Wahlert Catholic High School, 2005 Kane Street, Monday through Thursday, 8 a.m. to 4 p.m. and Fridays, 8 a.m. to noon, now through July 28, to pick up some apparel for next school year, free of charge.
Donations of uniform and spirit apparel will continue to be accepted throughout the month. All items must be clean and in good condition! You do not have to donate items to pick up items.

- Shop at the Booster Club Spirit Shop at Wahlert Catholic High School during office hours or during home fall athletics events.

Dubuque Community School District Busing
To qualify for busing, you must live more than two miles from your school (3 miles for high school students). Eligible families will receive a letter in August from the Dubuque Community School District indicating your bus route.
PLEASE NOTE: Holy Family students do not have busing on Friday mornings.
Transportation Reimbursement
If you qualify for busing but are not assigned a bus route, you may be eligible for transportation reimbursement. Submit the transportation reimbursement form before Dec. 1 for the full year. If you have a change in address, you will need to resubmit the form in the spring semester. Reimbursement payments are made in the fall of the following school year.
Jule City Transit
Families wishing to utilize The Jule for student transportation will need to purchase an Annual Youth Pass.

All school and system communications, including weather-related schedule change alerts, are delivered using ParentSquare.
Activate your ParentSquare account using the phone number or email address you provided in PowerSchool. Upon account activation, you will have access to adjust your notification preferences, access forms, school directories, make fee payments and more!

School Specific Information
Click on your child(ren)'s school(s) to expand the menu:
- Orientation for all 9th-grade students and for NEW 10th–12th-grade students will take place on Mon., Aug. 21, from 8:00-11:30 a.m. Students should report to the Wahlert gym in dress code.
- Chromebooks will be distributed on Monday, Aug. 21, to all 9th–12th-grade students who are new to the Holy Family system.
- School pictures for all students (except seniors) will be taken by Siebe Studios during the school day on Thursday, Aug. 24. Print the order form.
- Incoming 12th-graders are required by the Iowa Department of Public Health to have a Meningococcal vaccine before they start school.
- Incoming 9th-graders are required to submit a dental screening form.

- An Ice Cream Social for incoming sixth-grade students and their families will be held at Mazzuchelli on Wednesday, Aug. 16, at 6–8 p.m.
- Orientation for all Mazzuchelli students will be held on Monday, Aug. 21. Sessions will be held at 8 a.m., 9:30 a.m., 4 p.m., and 5:30 p.m. Families should attend at the time most convenient for them. No preliminary sign-up is necessary. After an opening presentation in the gym at each session, students will have school pictures taken, lockers will be accessible, and school supplies can be delivered to mentors' classrooms.
- Limited Edition Mazzuchelli gear will be sold during Mazzuchelli orientation.
- Class schedules will be provided during orientation and will be available for viewing on PowerSchool in mid-August.
- School pictures will take place during orientation. Purchasing pictures is optional, but all students must have pictures taken for school IDs. Dress code is not required. Print the order form.
- Immunization Requirements for incoming 7th-graders: Iowa Department of Public Health requires that all students entering the seventh grade have both a Meningococcal vaccine and a Tdap (Tetanus, Diptheria, and Pertussis/whooping cough) booster before school starts. org or your family doctor. - Fall Sports Parent Meetings will be held on Wednesday, Aug. 23 at the following times: 4:15 p.m. in the commons for all parents of Cross Country participants; 4:40 p.m. in the gym for parents of 7th-grade football players; 6 p.m. in the gym for parents of 7th-grade volleyball players. No preliminary sign-up is necessary to participate in sports. View athletic schedules here and return the Athletic Physical form to the school office.
- Mazzuchelli Parent Association - We welcome you to join our Facebook group for event information, volunteer opportunities, and school reminders. Please pay Parent Association dues through the "school fees" ParentSquare form (registration step 3).
