2018-2019 School Registration Information

Along with registration for the new school year come many individual items that require your attention.  To better streamline the process for families with students at more than one Holy Family school, this resource page will include all necessary instructions to finalize school registration and to prepare for the new academic year at each Holy Family school.

 

School offices are open throughout the summer, Monday through Friday, from 8 a.m. to 1 p.m. Families are welcome to stop by school offices for registration assistance at any time during those hours.  We request that families complete registration (steps 1–4) no later than Thursday, July 26.

The first day of school is Thursday, August 23, 2018.

 

1) e-Registration

Click here to complete PowerSchool e-registration. Locate the "E-Registration" icon in the lower left panel, and be sure to complete the process for each of your K–12 children.

3) Tuition Payment Options

Tuition statements were sent separately on Friday, July 6 (one per family). Please refer to the payment schedule provided with your statement to determine your first payment due date. If you did not receive the statement, please contact Kathleen Tracy, Tuition Coordinator, at (563) 582-5456 or ktracy@holyfamilydbq.org.

 

If you have not already done so, please fill out the electronic Tuition Contract & Payment Agreement (this form was also included with your tuition statement) and make your first payment by July 26. If you prefer to print and deliver your tuition contract, the form can be found below.

 

Tuition Payment Options

Holy Family Catholic Schools offers the following payment options:

  • Pay in full: Payment in full may be made by check, cash or credit card. If you pay by cash or check, you qualify for a one percent discount if the payment is made on or before July 26, 2018.
  • Semiannual: These payments are made at semester and may be made by check, cash or credit card.
  • Quarterly: Quarterly payments can be made by check, cash or credit card.
  • Monthly: Monthly payments are required to be a direct payment through electronic funds transfer (EFT).

Additional Information for All Families

Uniforms 

Uniform apparel is available for purchase online at holyfamilydbq.org/uniforms through the Holy Family eStore or Lands' End School. Samples can be viewed Monday through Friday, from 7:30 a.m. to 4:00 p.m. at the Holy Family Central Office located at 2005 Kane Street. Sizing varies greatly by item, so please consider bringing your child in for a fitting.  Lands’ End Fit Kits are also available at the Holy Family Central Office to help measure your student(s) to determine appropriate size. While orders may be placed at any time throughout the year, orders through the Holy Family eStore must be placed by Saturday, August 4, for delivery before school begins for the 2018-2019 school year. All items purchased through Lands’ End School will be shipped directly to your home. Please allow a turnaround time of 5-7 business days for Lands’ End purchases.  We encourage you to order early to have your uniforms before the first day of school.

 

Uniforms purchased through Lands' End between July 9 - 16, 2018 receive a free logo. No minimums. Stock up now and save!

 

Crested uniform apparel is only required at the middle school and high school levels, but is available in all sizes K-12. You may contact Tessa Petsche at tpetsche@holyfamilydbq.org or 563-582-5456 with any questions.

 

Please note that Second Chances consignment shop also carries a limited selection of gently worn Holy Family apparel. Open Monday - Friday, 9 a.m. to 5 p.m. and 10 a.m. to 3 p.m. on Saturday, Second Chances is located at 2600 Dodge St. Or call to confirm their current selection: 563-583-5474.

 

School Physicals & Medical Records

Physical exam records are required for early childhood students and secondary students who participate in athletics.  Elementary students (K–5) are required to submit a Health Records Form. FAll students should provide up to date immunization records.

Transportation

If you live within the Dubuque Community School District and more than two miles from your Holy Family elementary/middle school, or three miles from Wahlert Catholic High School, you will receive a letter directly from the Dubuque Community School District in mid-August (just before the start of school) indicating whether your child has the option to ride the bus (and his or her route) or elect transportation reimbursement when busing is unavailable.  Click here to locate the optional reimbursement form for those eligible.

 

Transportation reimbursement forms must be submitted directly to the Dubuque Community School District Transportation office by December 1 for first semester claims. ONLY ONE reimbursement form is required per family, per year, unless there is a change in a family’s transportation circumstances mid-school-year. Second-semester claims are automatically processed by the district if a claim is filed for the first semester. Families becoming eligible for transportation reimbursement after January 1 must file their claim form prior to May 1.

Jule City Transit

Families wishing to utilize The Jule for student transportation will need to purchase an annual youth pass. For more information, click here.

 

Nutrition Services

Those applying for free and reduced lunch plans need only submit one form per family, even if your children attend more than one Holy Family school (including Early Childhood).

  • Orientation day, taking place on Monday, August 20, provides the opportunity for families to visit with classroom teachers one-on-one to learn about curriculum information, classroom rules and procedures, special grade-level events and communication methods. Scheduling for these individual 10-minute sessions will open soon, in a manner similar to parent/teacher conferences. You will be contacted separately with the registration link and instructions. Students should bring school supplies to school on orientation day.

Resurrection Elementary 

  • A New Parent Meeting will be held on Monday, August 6, at 5:30 p.m. in the school cafeteria (enter through door #2). Children are welcome and babysitting will be provided. All NEW parents are requested to attend the meeting to learn about school procedures, to meet one another, and to meet members of the Resurrection School Parent Association.
  • Conference Day on Monday, August 20, provides the opportunity for families to visit with classroom teachers one-on-one to learn about curriculum, classroom rules and procedures, special grade-level events, and communication methods. Scheduling for these 10-minute parent/student/teacher Conferences will open on August 6.   • SchoolKidz supply kits, for those who ordered, will be available in your child(ren)'s classroom on conference day, Monday, August 20.

St. Anthony Elementary and Our Lady of Guadalupe

  • Please join us for a New Parent Meeting to be held on Monday, August 13, at 5:30 p.m. in our campus Literacy Center located in the St. Anthony building. Come and hear about our campus procedures and hints that will help launch a successful year. This will be a great opportunity for our new families to meet one another, meet some campus staff, and hear from our Parent Association.
  • Orientation day, taking place on Monday, August 20, provides the opportunity for families to visit with classroom teachers one-on-one to learn about curriculum information, classroom rules and procedures, special grade-level events and communication methods. Scheduling for these individual 15-minute sessions will open on Monday, August 6, in a manner similar to parent/teacher conferences.   • SchoolKidz supply kits, for those who ordered, will be available in your child(ren)'s classroom on orientation day, Monday, August 20.

St. Columbkille Elementary

Mazzuchelli Catholic Middle School

  • Orientation day for all Mazzuchelli students and parents will be held on Tuesday, August 21, from 8:00–9:30 a.m., 9:30–11:00 a.m., 4:00–5:30 p.m. and 5:30–7:00 p.m. Families should attend the time most convenient for them and no preliminary sign up is necessary. During orientation, students will have school pictures taken (dress code is not required for pictures), locker combinations will be assigned, and school supplies can be delivered. Families will have the opportunity to collect yearbooks, and SchoolKidz Kits (for those who purchased), purchase spirit apparel and sign up for volunteer committees.
  • 1:1 Chromebook distribution/replacement – All students new to Mazzuchelli, including all incoming sixth-graders, will be issued Chromebooks during orientation.  New Chromebooks will also be issued to seventh-graders this year as part of our regular technology update. Please have those students bring their current Chromebooks to orientation for replacement. Students keeping their Chromebooks this year (current eighth-graders) will have them replaced at the start of next school year.
  • Activities and Athletics (seventh- and eighth-graders only) – Students can sign up for Show Choir, Mock Trial, Opus Honor Choir and fall athletics (football, volleyball and boys and girls cross country) during orientation.   • New immunization requirements for incoming seventh-graders – The Iowa Department of Public Health has made changes to the Iowa Immunization law that requires all students entering seventh grade have both a Meningococcal vaccine and a Tdap (Tetanus, Diptheria, and Pertussis/whooping cough) booster before school starts. Wahlert Catholic High School

  • Freshman orientation for incoming ninth-grade students will take place on Wednesday, August 22, from 8 a.m. to 1 p.m. Students should report to the Wahlert gym in uniform, with a pen/pencil and paper. There will be a social dance for ninth-graders from 1:00–2:30 p.m. in the cafeteria.
  • 1:1 Chromebook distribution/replacement will take place during freshman orientation for all ninth-graders. Ninth-graders who have not already returned their Chromebooks should bring them to orientation for replacement. All 12th-graders will also have their Chromebooks replaced. Those who have not already returned theirs can bring it to Mazzuchelli on Tuesday, August 21 or have it replaced in the technology office when school begins.
  • School photos will be taken by Siebe Studios on Thursday, August 30.
  • Activities and Athletics – practices begin August 7 for those participating in fall athletics (football, volleyball, boys and girls cross country, boys golf and girls swimming.) Click here to print the Student Athletic Physical Form.
